The Lasting Power of Attorney (LPA) is a legal document that allows a person (donor) aged 21 years and above to appoint one or more persons (donee(s)) to make decisions on his behalf should he lose the mental capacity to do so. This document needs to be signed by a Certificate Issuer -an accepted Medical Practitioner (doctor) / lawyer or psychiatrist– who will certify that the donor understands the implications of making an LPA.

How to make an LPA

Step 1: Appoint Your Donee(s)

A Donee is someone who is aged 21 and above, who will make decisions for you when you lose mental capacity.

Step 2: Draft Your LPA Online

Login to the Office of the Public Guardian Online (OPGO) via Singpass to draft your LPA.

Step 3: Visit a Certificate Issuer (CI)

CIs are accredited Medical Practitioners, Practising Lawyers and/or Registered Psychiatrists.

Step 4: Digital LPA is Ready

When the digital LPA is accepted and registered. Donor and Donee(s) will be notified of the status.

Making an LPA allows you to appoint one or more persons (donee (s) ) who can step forward to act on your behalf if you lose mental capacity to make your own decisions. Your donee(s) can act in two broad areas – personal welfare and / or property and affairs matters.

You need to be at least 21 years old, and have mental capacity. You will need to register the completed LPA Form 1 and other relevant documents with the Office of the Public Guardian (OPG).

To make an LPA:

1. Choose your Donee(s) and decide what decision powers to grant your Donee(s)

2. Draft your LPA online

3. Visit a Certificate Issuer (CI) to certify your LPA and sign using Singpass digital signature

4. Donor and Donee will be notified of LPA status via SMS / email when the digital LPA is accepted and ready
